Why aren't my moderation settings working as expected?

That is not consistent with what he’s seen. Via email, he told me:

Regarding the password protected posts, it appears as though if the user is subscribed to receive comment notifications, they will only see comment notifications on password protected posts once they login and view the post. This verifies that they know the password and therefore triggers the comment notifications to come through. If they haven’t logged in and viewed the post before a comment is made, they will not receive notification of that comment. Does that make sense? Can you ask them if they received notifications for the last two comments on the password protected post?

What you described here is not relevant, as the user made the 2nd of 14 total comments on my post, which he had to use the password to do.

Is the user only experiencing problems with receiving comments from your blog or from others as well? If it’s the latter, could you ask them to open a new thread for support? That will help prevent having to use you as a middle man to ask questions back and forth!
Regarding the moderation settings on your blog, have you been receiving moderation emails for your recent comments? I don’t see a moderation email for the comments on 5/1 or 5/2.

Below, I am attempting to paste a table indicating whether each comment in my protected post has required my manual moderation approval. It seems I received no emails at all for several of them–perhaps I approved them via the Android WP app before the email was sent–thus no email was sent?
The “Email” column (2nd-to-last) indicates whether I received a “please moderate” (MOD) email or just a notification (notify) email of an auto-approved comment. The last column is me attempting to fill in the gaps by memory of which other posts I had to manually approve (“yes”). Blanks in both columns are then unknowns.
“Lvl” is the nesting level. “Who” is an abbreviation of the username.
