Why can't I see the complete new toolbar?

I’m trying to add images to a post I’m currently writing and the new toolbar doesn’t load at 100%. That is to say, I don’t have the “Add Media” “Add Poll” and other 2 buttons that are supposed to be shown above the non extended toolbar.
I cannot add images!!! It only gives me a choice, below the text editor, to create a gallery, but then such gallery doesn’t even show along with my post.

Thanks a lot for your super speedy answer! I’ll try everything and will read all of the links you showed me. However, I have to agree with some fellow users: I’m not diggin this new interface.

I am having all kinds of problems. When I log into my blog, fourwindowspress.com, the tools that I used to have on the top of the page do not load. I’ve slowed down on posts because, though I can add new posts, it’s always a lot of work to make it happen. Also, when I and my wife try to comment, we can’t just comment and enter the comment, we have to login every time on both our own blog and on other blogs. A couple of times another screen challenges us to prove we are Thomas Davis, and a couple of other times it informed us we were not logged in and erased the comment. What is going on? And how do I fix it?
