Why do people’s comments end up in the wrong post?

Hello all, not sure if this is the same help area for the .org sites as well but thought I would ask here.
Quite simply, peoples comments are being attributed to the wrong posts. They show up on the dashboard as being inputted into the wrong posts as well. Anyone any idea here?
