Why does contact form show my info instead of blank fields?

  Contact form pre-fills with my email etc. I want the fields to show up blank until viewer fills them.

  The blog I need help with is: (visible only to logged in users)

  That is just because you are viewing your site as a logged in WordPress.com user. The system autofills that info in. If you log out, or use a private/incognito browsing window, you will see blank fields.

  Does that make sense?
