Why does my personal information populate in the contact us form?

  When I put the contact form into my blogs MY email address and personal information becomes public on the form. What do I do to correct this. This is on all of my blogs. I had this problem before and had to call you.

  log-out – when you are logged in WordPress.COM remembers some of your info and saves you some typing – if I visit your site MY info will be filled out on your forms – not your info
