Why is the post title wrong on Facebook, Twitter and Tumblr?

I was writing a post with the initial title “Adventures In Erotic Graphs” and at one point I edited the post title, changing it to “Adventures In Graph Innuendo”.
I worked on the post for a while after that, previewing it numerous times before I published it, and each time I previewed it the title was correctly showing as “Adventures In Graph Innuendo”.
When I published it the correct title appeared on WordPress.com as well, and the permalink was correctly appearing as: http://fishandbicycles.com/2014/12/14/adventures-in-graph-innuendo/
However, the original title appeared on my connected Facebook, Twitter, and Tumblr pages.
I find this really problematic, not being able to trust that changes I make will stick everywhere they should, I have no idea even how I would go about preventing this from happening, and of course I’m unable to correct the posts on Facebook, Twitter, and Tumblr.
I’d be grateful for some assistance. Thanks!

Currently, for that post, it looks like the Custom Message set for the social media posts is still set to the wrong title:
From the Edit Post page, you should be able to see that here:

Before you publish a post, you’ll want to make sure that is set correctly so the correct title appears!
