Why is the video appearing twice when I have only embedded 1 Youtube shortlink?

  I am having an issue where the Youtube video I am embedding is appearing twice on all new video posts I complete. It is fine on my previous posts but now everytime I embed a new video link it appears twice. You can view the issue below:

  Anyone have an ideas/suggestions?

  WordPress.com and WordPress.org are completely separate and have different logins, features and run different versions of some themes with the same names http://support.wordpress.com/com-vs-org/
