Why is there Delay in posts appearing in Google Reader (and other readers)?

Can anybody explain why there is a delay between when I publish a post and when it is visible on Google Reader and Blogger Dashboard.
Several of my Blogger followers have remarked that the posts are delayed and sometimes come several at once despite my only posting no more than once per day.
Today, even after 6 hours it is still not appearing in Google reader.

The feed doesn’t go out until someone goes to the blog itself, rather than reading it on a reader etc, if I’m remembering correctly.

But if that is the case, how do people know you have posted and go to your blog if it doesn’t appear on the reader?

People who read on the Reader rarely go to the blog. People go to the blog because they prefer to read it on the blog or they check every day or they come via a search.

The post goes out via the feed instantly.
The delay of a post not appear until someone visits the blog only applies at scheduled posts.
