Why isn't mail chimp subscription button popping up on wordpress blog post?

  I added my mail chimp subscription pop up form HTML code to the HTM form in my wordpress blog (trying this for the first time) and the subscription pop up is not popping up. Why isn’t it?

  The blog I need help with is: (visible only to logged in users)
