Widget Help – Local Harvest Widget

Does anyone know how I can get this widget on my wordpress site? Do I use a text widget to incorporate this?

There would be a zip code black form that you can type in your zipcode and then you will get directed to Local harvests page of all the farms in your area.
The blog I need help with is: (visible only to logged in users)

Sorry, forms are not allowed at wordpress.com for security reasons.
You could use the third option on that page and put the graphic that would link back to their site where visitors could then search.
