Woocommerce order confirmation via email but none in the system

Hello forum,
my customer has an internal employee shop where his employees can buy different gifts with their points. They get these points e.g. for birthday, employee of the month or something similar.
The Woocommerce shop only runs with the “Points and rewards” system.
So far it works quite well. However, it has happened a few times that an employee has bought an item with it’s points, the office has received an order confirmation by e-mail, but there is nothing in the system. Also, no points were deducted for the purchase.Does anyone have an idea of what that could be?
