Woocommerce order missing line items

I recently had an order come through that had no line items, but 2 shipping methods added. The shipping methods are attached to a product (the same product) but no actual products show up. It was not uploaded manually and we’ve never seen this happen before. There are orders after it that are fine. We cancelled and refunded the order and there are no other notes that indicate any activity on it such as removing a product after the after the fact.
We use stripe for payments, we have octolize shipping plugins installed but I think for this particular site (its in a multisite install), we are just using some basic flat rate shipping. None of the other sites in the multisite have had any problem. The product in question is in stock for each variant and all variants have pricing around $90. We have no coupon codes set up.
