word truncated between comment being made and me getting the comment

The commenter, who I know personally told me today that where the word “s” originally appeared three times towards the end of her first comment (it’s now changed to “%s”), she had originally written “accidents” in all three places.
Then when I got her second comment asking for it to be changed to “%s”, I did as she requested. In this case also she said she had written “accidents” in her second comment, not “%s”.
I wonder whether this could be anything other than “big brother” hacking into these communications and making these changes that don’t seem like they could be caused by anything else?
The commenter said she thought I had deliberately changed her word. Of course I didn’t.
Has anybody out there experienced anything of this sort, or does anyone know how this could have happened?
