WordPress custom form that shows data on a specific page?

Hi all!
Before anything, I beg all of you to read this carefully, as I am still new to WordPress in general, so I might have trouble explaining my problem.
I posted this same problem on the support forums, and twice on yahoo answers, but to no avail. I got no answers at all. Therefore, I am posting this here, in hope that somebody could help me.
I am a teacher at a high school and I am planning to make a site accessible only by me in order to aid me in my job.
What I was planning to do is to make a private site containing all the information necessary for me: school contact info, schedule, my daily lesson preparations etc.
The most important thing I was thinking of doing is to make a form-like page for each individual student of mine, so that during every lesson I can choose a student, write a comment and enter a grade if I wish, and then click save. I would like then that those information show on a specific page under that student’s name, and that later I can see all the notes and grades of that student on that page. That way I could track his/hers progress
For example, If I have a page called e.g. “Evaluation”, and i open it, I would like to have a form similar to this scheme:
– [dropdown menu] “Choose class” (since I teach several classes)
–>I choose e.g. class 3B– [dropdown menu] “Choose a student” (each class has 10-20 students)
–>I choose a student “Name Lastname”– [dropdown menu] “Choose evaluation element” (e.g. grammar, pronunciation, verbal skils, etc)
–>I choose e.g. “Grammar”– [radio buttons] “Choose a grade” (from 1-5 in my educational system. 1 being the worst, 5 being the best)
–>I choose e.g. grade 3– [text field] “Type a note” (optional)
– [button] “Save”
After the submission, I would like to have the possibility to navigate through the standard site’s menu to the class 3B, to pick the student Name Lastname and after I open that page, to see the date of the evaluation, evaluation element, grade and optional note.
Is there a way to do this or is there a plugin that can be modified to do this? Does anybody have any advice on this matter?

Can it be done on a WordPress-powered website, and could you at least give me some guidelines where and how to search for the solution for my problem? Googling “advanced forms for wordpress” and/or “custom forms for wordpress” or similar keywords usually redirects me to various plugins that are not what i am looking for.
Thank you once more, and since I have clearly gone to a wrong forum, (sorry for that!) I will not post any more to this topic after your reply, unless you require some other information from me.
