WordPress.com blog transfer to wordPress self-hosted

I have a wordPress.com blog but I am getting a self-hosted one (because the wordpress.com has some limitations).
Is it possible to keep my blog´s name and to import the old one?

No you cannot keep the .wordpress.com URL. You hire a web host, purchase a domain, set up a wordpress.org install, install a wordpress.org theme and plugins. Then you can export the content out of the wordpress.com blog and import it into a wordpress.org install.
