  I am having trouble understanding whether or not the WordPress.com Premium includes a free domain for more than the first year. Talking numbers, does the price of WordPress.com premium with a unique domain go up from $99 the first year to $117 (including the $18 domain registration) for every year following that?

  No. WordPress.com Premium is $99/yr and every $99 payment includes the cost of the domain registration. You do not pay the $99 + the domain registration fee.
