Would like to have a custom form for blog

I have a blog with wordpress.com. It is for a moving business. I need and want a form that a potential customer can fill out which will allow me to give a free moving estimate or answer a question. Basically I want a “request for info” form on the blog. Is there such a plugin or other means to gets this created? Thanks

You cannot create a custom form on WordPress.COM. (You can do this on self-hosted WordPress.)
But, you can use the regular contact form in place of the comment box you currently have on the contact page. It basically does the same thing, but doesn’t post the reply on that page.
Make sure to turn of commenting on the page if you decide to use the contact form instead.
