WPForms Issue

I have three forms on my site that use a mix of question types. The were displaying fine to start with until a few days ago when any questions that have checkboxes have disappeared. I’ve included a screen shot of what I’m seeing. There should be a line that asks why type of phone do they have above the “Cell, Home, Work” boxes. But boxes like the Email box and the Phone Number box are fine.

I can confirm that I see the same issue on your Volunteer page. However, since you’re using WP Forms inside Elementor (both of which are third-party plugins), we are unable to troubleshoot this. Have you had a chance to contact WPForms support regarding this? You can post on their support forum for help with the free version.
