WPForms (Only one working)

Off my website (purposedriventherapy.com) I can only get one of my created WPForms to deliver feedback. My other forms are not going anywhere. I duplicated the ‘Contact Us’ form that is working; but those duplicates still aren’t delivering anything I can see. The ‘Contact us’ form is submitting to the admin email & is showing up in the feedback on the dashboard for the website. None of the others are though. help!

I am afraid you are in the wrong forum for your query.
It is likely your site purposedriventherapy.com is using the open source WordPress software. Getting to https://wordpress.org/support will help you find both the support and documentation for the version of WordPress you are using. Alternatively, you can take this question directly over to the WordPress.org support forums:
