Bio
Our mission is to improve the health of persons living in the Uptown, Edgewater, Rogers Park and Albany Park communities of Chicago – and enhance the health status of these communities overall-through the provision of affordable, quality, multilingual and culturally competent health services designed to prevent illness and promote physical, mental and social well-being.
We operate five facilities on Chicago’s north side:
Heartland Health Center – Rogers Park
Heartland Health Center – Uptown, Heartland Health Center – Roosevelt High School
Heartland Health Center – Senn High School
Heartland Dental Center – Uptown
