エントリーフォームのエラー

    エントリーフォームを入力すると下記の2箇所がエラーとなります。
    全く素人で申し訳ありませんがおしえてください。
    １．フリガナのところは未入力ですが志望動機に入力をしたテキストが表示をされます。
    ２．携帯電話の入力フォームに数字を入れるとこのように表示されます。携帯電話：[your-your-tel]
    よろしくお願いします。
