Adding a page for survey

  I want to add a page for survey, such that whatever typed in the text boxes onnthat page will be sent to my email. But I am a beginner and dont known anything about .php pages (script). I don’t have my laptop with me now.

  1. Can I create such page using android device or using wordpress application ( for android)??? If so plzz tell me.

  2. Can I use HTML for the purpose???? If so, plzz provide me the link for it.

  3. Plz provide the page-link or tutorial video link for this purpose
