Adding decision tree to WordPress page.

    I want to add a decision tree (If it even is called a decision tree) to my site. The idea is creating a recommendation system for my clients based on their needs. The client will answer 3-5 questions. Based on the answers they provide I will recommend a set of products suitable for their use case. Is this possible via plugins on WordPress? Or is there another way of doing this?
