Blog subscribers previously moved from wordpress.com to wordpress.org disappear

I am seeking support on behalf of my client:
My client previously requested to have her blog followers from her wordpress.com site (http://neilyonnutrition.wordpress.com/) transfered to her new wordpress.org site (http://neilyonnutrition.com/). That request went through successfully, but now those blog followers have disappeared, and there are only two blog followers that subscribed since the site went live that show up in the Jetpack stats.
Could you look into this for me? Thanks.

I believe the email she uses for her wordpress.com account is jenniferneily [at] yahoo [dot] com

This is the result of looking for a username account https://en.forums.wordpress.com/profile/neilyonnutritionProvided the blogger in question has installed and activated the JetPack plugin http://jetpack.me on her WordPress.org install, and posts the relevant URL(s) into a forum thread, Staff will do the transfer of both subscribers and stats for here. http://en.support.wordpress.com/moving-a-blog/moving-your-subscribers/#wordpress-com-to-self-hosted-site She needs to login now and post into this thgread as I have tagged this thread for their attention.

It looks as though she requested that her subscribers be migrated to http://neilyonnutrition.com/blog/. But at the moment, I see Jetpack connected to http://neilyonnutrition.com. That difference is responsible for the missing subscribers. The subscribers were moved to the location that was requested, but then someone set up the blog again at a new location.
This can be fixed, but your client will need to post on this thread herself and make the request.

Please move any and all blog subscribers from http://neilyonnutrition.com/blog and http://neilyonnutrition.wordpress.com/ to the main wordpress install at http://neilyonnutrition.com. Sorry for any confusion and thank you so much!
