can i design a website survey where respondents can upload video responses?

    I am workign with the Deaf community and they use signed language
    I woudl like to run a simple online survey.
    each page contains a question and say 5 possible responses.

    For eample, how would you rate your overall experience(s) with the hospital and/or A&E staff?

    o Not applicable (never used)

    o Prefer not to answer

    o Very Unsatisfactory

    o Unsatisfactory

    o Neutral

    o Satisfactory

    o Very Satisfactory

    Why? (Please specify): _______________________

    Next to this question is a link for a video version. Click and a video starts where a sign langauge interpreter signs the question.
    at the bottom is another link where the participant can upload his or response.
    participants would have the option of recording a reply in signe dlangauge with their webcam and uploading to the webpage page.

    then they move on to the next question. repeat.

    This is the design that I would like to set up.
    is this an option on WordPress?
