If a nurse comes to the site and would like to ask a question for other nurses, how does it get put into the Nursing category?

Categories are meant to be the main sub-groups of your POSTS.
Unless you deactivate it, a comment section is available under each POST or PAGE you publish.
Perhaps you should consider creating PAGES with the headings you now have as categories.

Comments do not have categories as such, only posts have categories. Comments are responses to posts. You could set up some static pages named for the categories you expect questions in and they could post their questions there as comments I guess.
It actually sounds like you are looking for a forum rather than a blog.

how nurses in Kingdom can interact with each other, is there any specific platform????. I would like to bring nurses together in Kingdom to share their knowledge, experience, skills, problems and concerns. Are we allowed to do that as foreighn nurses???, I want to bring together local, regional and native nurses together with the foreighn nurses working here
