contact form includes my own info

    When a contact form is added on a page, name and email fields display my own username and email address. I do not want to show my info and there fields should be where readers enter their info. How do I fix this?

    You don’t. It’s not broken. Because you are signed in, the form can detect and autofill this info. It is not visible to anyone else. When someone else goes to the page a) if they’re not signed in those lines are blank b) if they are signed in those lines are filled in with their info.
