  There are two email addresses to change for notifications.

  Account-specific notifications go to the email address at Users -> Personal Settings, and blog-specific notifications go to the account at Settings -> General.

  I have only my (administrator) email address in both places. I have it set up to approve comments before publishing. When I approved the comments, emails went out to all users with the comments I approved. This is the first time this has happened and I have not changed my settings for anything on the blog. Not sure what is going on.

  Some of your users may be subscribed to receive notification of new comments via email. This is actually a rather common thing.
