  Can i put survey form on my web page & also can i get the collection of email Id from the Newsletter? & if yes then with which package should i go for. Personal, premium or Business.

  You can embed a poll (similar to survey) from Polldaddy directly to your posts or pages:

  Create a Survey, Poll, or Quiz

  and you can also use the popular Google forms:

  WordPress.com does not offer a tool for creating your own custom newsletters; however, if you would like to offer a newsletter subscription, you can use a contact form that interested readers can fill out to be added to your newsletter list. Then, you can send your newsletter out from your own email client or newsletter service.
