Form Plugin that lets the user add rows

    I need to Find a plugin that lets the user add a row of fields after filling out the first etc. So, for instance, the user is asked to put in a product code and description then clicks add new product and a new row of fields is shown and when done he/she is able to submit all his answers at one go. Please Help!
