form setup not working / or being over riden

Good day
I am using a theme called inspired by Curt bought from Themeforest.
the website is http://www.bml-llc.com
the theme came with a contact me form. I activated Jetpack and used the icone on the contact me page setting up a new form. the code on the page reads: [contact-form] [contact-field label="First and Last Name" type="name" required="true" /] [contact-field label="Email" type="email" required="true" /] [contact-field label="Phone number with Country Cod" type="text" /] [contact-field label="Age" type="text" /] [contact-field label="Male" type="checkbox" /] [contact-field label="Female" type="checkbox" /] [contact-field label="Medical Condition and/ or Desired Location" type="textarea" /] [contact-field label="Comments / additional information" type="textarea" /] [contact-field label="I have read the TERMS & CONDITIONS and HEALTH AND PRIVACY STATEMENT in the BML website" type="checkbox" required="true" /] [/contact-form]
but it does not update and shows the old form.
Jetpack came preloaded with the Theme and i just activated.

It sounds like the theme’s Contact page has a built-in form (probably in the template) that can’t be altered, but I recommend checking with the theme’s vendor about this.
You should be able to add a Jetpack contact form to any other page without issue.

Nope:
set up a test page and cut and pasted code above and in the view it was the same old contact setup. Have a question up on the theme support site as well but Curt is not always helpful.

Ok, I imagine the theme is overriding it then, which we have no control over.
The best way to test would be to see if you have the same trouble in the Twenty Eleven theme.
