Forms do not submit.

I’m using Form Maker Pro. After moving my site from development to production, forms created using FM do not submit. Nothing is stored in the database and no email is sent. The submit button does perform the form validation, but the submission never occurs.
Web Dorado suggested I check my .htaccess file, specifically PHP POST and GET rewrites. The development site did not even have a htaccess file and submissions worked perfectly.
Being new to WP, I’m lost as to where to look.
Any insight into what might be causing this issue is greatly appreciated.

Your site ms-na.org is not hosted by us (WordPress.com). It is using the self-hosted version of WordPress and it is hosted by BlueHost. Therefore, you need to contact them regarding your issue.
