Getting premium

  I want to go premium, I already have my blog and my question is, if I go premium then i will have to pay $99 dollars per year but do I have to pay extra $18 for my site to move to a .com domain? What if i want to have more than one wordpress blog? WIll I have to pay $18 extra for each blog?

  The blog I need help with is: (visible only to logged in users)

  I’m confused a little bit, tell me, if i pay $99 a year do I have a domain included? Or do I have to pay extra $18 ? ($117 in total for premium + .com domain)
