GiveWP Donate button stopped working

My website (cincinnatifriends.org) has been successfully using the GiveWP Give Donation plug-in for many months, but today a user complained that the button is no longer working. I see from the Donations page that the last successful donation was December 6. Sometimes when you click the button, you get a Page Not Found error, other times you get about:blank#blocked. I haven’t made any changes to any of the forms, although we were having some problems with our other pages not using SiteOrigin Page Builder, and I fixed those issues. In doing so, I did convert the various Donation pages, but converting them back to use the standard WordPress editor did not fix the problem with the Donate button on our home page.
I tried copying the Shortcode again and pasting that as the Destination URL for the SiteOrigin button, but that didn’t fix the problem.
It’s been a long time since I set this up, and don’t remember all the details about how I did it. I can’t figure out how to get (non-premium) support from GiveWP. Any assistance anyone can provide would be greatly appreciated!
