  I want to have a form that my visitors can print off – right from my website

  how do I put a form on my website that my visitors would be able to print off?

  It is an informational and registration form to fill out and to mail in.

  I tried uploading it as a pdf file but it wont work. if I have the pdf file in my library can i create a link to that file?
