How do I add a file uploader to my custom form?

  Please help me add a file uploader to a custom form. I would like for site visitors to be able to submit a file to me (admin). This would ideally be a function of the custom form (found on the RENEW Application page) but can also be separate if needed.

  At this time, we do not have a way to allow visitors to upload files, but I have noted your request for consideration.
