3. Exactly which username account registered the underlying .wordpress.com blog and purchased the upgrade for it?
4. When was the upgrade purchased?

When you say you want to start over, what has gone wrong? From looking at your account I can see you have a domain included in your plan; we don’t advise cancelling a domain if you want to use it again in the future, as there is no guarantee you will be able to get it back again.
If you let me know the problem you are having, I can help you sort it out. However, if you have already made up your mind and are happy to cancel everything, you can do so from your Manage Purchases page:
