  I tried to add a contact form and when I preview it reflects all my information. I don’t want to publish until I know it will be blank boxes that viewers can type into and that it will not show my information publicly.

  The blog I need help with is: (visible only to logged in users)

  You are seeing that because you are viewing the contact form as a logged in WordPress.com user. If I viewed your contact form, I would see my info pre-filled.

  If you log out of your WordPress.com account and view the page, you will see blank fields.
