I had a mailchimp signup form on my website. I wanted to change it to double opt-in, so I went to settings and clicked on double opt-in. When I checked my website the signup form had disappeared. How can I get it back?
The site I need help with is lindajspielman.com. It is not hosted by WordPress.com, but it is connected with Jetpack.

Hello, I’m sorry to hear that you are having trouble with your website. However, your site is not hosted with WordPress.com. It is a site using the open-source WordPress software (from WordPress.org) but hosted with Bluehost.
Because WordPress.com and WordPress.org are two entirely separate entities, we cannot access files or data for sites that are hosted elsewhere, so WordPress.com staff can only assist with sites that are hosted on our servers.
