Moving subscribers from dychadziennie.wordpress.com to dychadziennie.pl

  Hello,
  I would like to ask whether there’s a possibility to move my subscribers from wordpress site to my self-hosted one (dychadziennie.pl) I also use wordpress theme there but as for now there is no possibility for my subscribers to get informed by new posts on my new site. I informed them in the last post on my dychadziennie.wordpress.com site but the problem is still relevant. Furthermore, how can I be able to subscribe other wordpress bloggers from my new site? Is it possible?
