My online registration page keeps reidrecting to home

In the past hour, my online subscription page (www.maxpotentialdogs.ca/online-obedience-class) is redirecting back to home. When I go to edit the page, it looks exactly like my home page. I have no idea how this has happened but need to reverse this as fast as possible. Thank you.

I’m afraid we won’t be able to help you here as your site is hosted on Siteground, not on WordPress.com.
Because WordPress.com and WordPress.org are two entirely separate entities, we cannot access files or data for sites that are hosted elsewhere, so WordPress.com staff can only assist with sites that are hosted on our servers.
