  Years ago I started an account with WordPress.
  I’m coming back now to create a website for a new project.
  Do I start a new domain name through WordPress?
  Do I get an email address to forward email (or can I set up a new account in my Apple Mail app?
  How do I find out if they’ve been billing me all this time?

  Correct account: Yes

  You can start a new site, and choose an optional custom domain name, at any time from https://wordpress.com/start

  Do I get an email address to forward email (or can I set up a new account in my Apple Mail app?

  Not automatically, we have some recommendations at https://wordpress.com/support/add-email/

  How do I find out if they’ve been billing me all this time?

  We have no history of purchases, transactions, or billing activity of any kind under the account you’re currently logged into.

  If you has purchases/plans though, you’d be able to see that at https://wordpress.com/me/purchases
