  I have written HTML and CSS codes to create a form but I have no idea as to how to make it upload what has to be uploaded into the DB

  Hi there, can you post the HTML here between code tags so that we can take a look?

  If it uses the <form> tags, then those are not allowed here at WordPress.com. To add CSS, you would have to upgrade to either the WordPress.com Premium or Business plan, which include Custom Design and the ability to add custom CSS.

  What sort of form are you wanting to add? We do directly support Wufoo, Jotform and Google forms, and there would be no upgrade required to use those. You can click through to the individual form support pages for more information on each.
