product add-on plugin

I’ve just got this plugin today, works great but for some reason its duplicating the text field information in the basket. Also the product is now adding the value of the product and price of the service together then adding 20% vat on to the full product.
Is this something you can help with? or is it an issue with the plugin and my website?
