Removal of self-hosted blog from list of my blogs

  I want to entirely remove an expired link and any other referral to a self-hosted blog from my account profile (Blog Name uniBlock, Link http://www.uni-block.de/). Please advise me.

  The blog I need help with is: (visible only to logged in users)
