Remove self-hosted site from WordPress.com/sites

I notice that WordPress.com’s Sites section have a new UI. However I can’t find any clue, where the Jetpack disconnect link location is.
I want to remove one of my self-hosted blog from WordPress.com Sites.

Did you try using this guide? http://en.support.wordpress.com/disconnect-jetpack-sites/ If you did and had no success then post the URL you want deleted here, and type modlook into the sidebar of the thread and Staff will delete it for you.

That is the problem. WordPress recently release new UI for WordPress.com/sites section, read in their blog here http://en.blog.wordpress.com/2014/10/21/a-new-my-sites-section/
I can’t find any “disconnect Jetpack” link in new interface..

Is this a currently active jetpack site? The instructions you were probably looking at do not work for Jetpack sites that are currently live. If you want to disconnect active Jetpack sites, you will need to log into your self-hosted site’s Dashboard and disconnect Jetpack directly from there.
