Setting the contact form email subject line from visitor's input

Hello,
I’ve searched for hours and haven’t found a solution for this issue. Basically, I have a simple contact form like this:
Name
Email
Subject
CommentWhen a visitor fills it out, I receive an email with the subject “[{MY NAME}] Contact”. What I want is for the subject line of the email that I receive to be whatever subject the visitor entered into the form.
Is this possible?

You can have a specific subject line – that can be coded into the contact form – you can have more than one contact form on your site ( I have two or three I think all on different pages) – my pages are hidden with a custom menu
You can have different contact forms – don’t think you can have changes based on what a visitor types in.
Have different forms if they are entering from different areas – requesting information on different subjects
