Subscription Form Issue

  I am trying to add mailchimp subscription forms to two landing pages. The forms should look the same, but once filled in and submitted should also save information about the landing page (e.g. URL) it came from.

  Any suggestions on how to do it?

  The blog I need help with is: (visible only to logged in users)
