  I have added the universal code to the footer but does not display anything on the website for Pop-up Form

  I checked your site and it appears you’re in the wrong forum. Although your site is connected to WordPress.com via Jetpack, it is using the open source WordPress.org software. You can seek help with Constant Contact in these forums:
