What file to edit?

  Here is what I have done so far.

  As you can see, I added a link to a form to the single post template file. If a video is not working it takes the user to a form where they can report a broken video.

  This is only displayed on the individual content pages. I want to add it to the search results and category pages on each piece of content automatically.

  I would like to have it show up right above the bookmark and share section. Where would I put the code for the link in?
